CALL, WRITE, E-MAIL, ORDER ONLINE or FAX Include the following information:

1. Purchase Order Number or name of person authorized to purchase
2. Your "ship to" and "bill to" address
3. Description of each item on your order, the part number, quantity, and finish as required.
